 Here we are in Miami where the worst thing is the vitriol of the local and national politicians.

I think 

of the winds hitting the fragile houses and farms of Pinar del Rio in Cuba as I write this and the fear of the people in Louisiana to Texas awaiting the hurricane 

we have been spared here in Miami.

In Cochin, they are afraid to go out .

My favourite hotel is closed and I am dying to board Qatar Airways to go anywhere ..

what is my suffering compared to those whom I love in so many parts of the world 

The lady who squeezes juices and sell them for a dollar by the river in Siem Reap ? I cant even call her to console her ..

The sun is rising over the ruined temples of Angkor and the hundreds of tuk tuk drivers await another day of hunger 

while I sit here sipping my wine

We in the west are heartless 

we mourn the loss of convenience 

they mourn the hunger that has eradicated their appetite 


Come to me, I will hold your hands 

The next time I see you, I would double my warmth for you 

the shopkeepers of Fort Cochin who cant even pronounce my name 

I am with you brothers and sisters, my heart is there for you and my tears accompany yours 

the pregnant woman whose husband is awaiting repatriation in Kuwait 

simple pleasures taken away from simple people , this is injustice, you cruel covid

No Mullah No Priest No Rabbi No Imam can justify or find a sentence in their sacred books 

throw out your starched white clothes and hats and share your food, Halal or not, with the poor people, your people, in Yemen or Indonesia or Sudan or Morocco..

REMEMBERING WHAT MY INDIGENOUS TEACHERS TOLD ME ABOUT DIABETES CARE AMONG THE NATIVE PEOPLE

I will never forget the words of the elder, Adeline W.

Here we were sitting at her modest home in the settlement of Indians out of the bling and business of Des Moines (always think of the American humorist, Bill Bryson who published a book called The Fat Girls in Des Moines).


In the short period of time we knew each other, Adeline put me on the right path to taking care of Native Americans and later, Indigenous peoples of other continents.

We can see through you, so no point in telling us, you care for us, if you do not really care for us. We feel better when we know that we really care for us.

To which my teacher, (dr) Brown added: respect Indians, do not judge them and if possible love them.

When a health care person, does not matter whether he/she is a doctor at a Los Angeles private clinic or a peer to peer educator in Cambodian capital’s slums, what is important is that he or she conveys to the patient, his/her genuine care. It has to be genuine. No one is interested in how much you may or may not know, but they do care HOW MUCH THEY FEEL YOU CARE..

I am reminded of a poem by Pablo Neruda

Now what did the doctors in Chiang Mai find? What influenced the patients with Type 2 DM want to take their advice along with their medications?

 

Medication adherence in Thai patients with diabetes requires support from both the health care providers and the family. The patient’s perception of the doctor’s concern creates greater patient trust in the health care team. This trust, along with family support, helps deepen patients’ understanding of the disease, accept the chronic nature of their disease, and engenders a positive attitude towards taking medication that can improve medication adherence.

Feigning an interest in the patient without your heart being in it, is like those big billboard signs one sees all across USA of hospitals and Doctors advertising how much they care for their patients!  A big lie and in big letters! YET ONE OTHER REASON TO AVOID DRINKING COCA-COLA AND ITS FIZZY DRINKS COUSINS ..

Confederate Colonel John Pemberton, wounded in the American Civil War and addicted to morphine, also had a medical degree and began a quest to find a substitute for the problematic drug. In 1885 at Pemberton's Eagle Drug and Chemical House, his drugstore in ColumbusGeorgia, he registered Pemberton's French Wine Coca nerve tonic. Pemberton's tonic may have been inspired by the formidable success of Vin Mariani, a French-Corsican coca wine,  but his recipe additionally included the African kola nut, the beverage's source of caffeine.

It is also worth noting that a Spanish drink called "Kola Coca" was presented at a contest in Philadelphia in 1885, a year before the official birth of Coca-Cola. The rights for this Spanish drink were bought by Coca-Cola in 1953.

In 1886, when Atlanta and Fulton County passed prohibition legislation, Pemberton responded by developing Coca-Cola, a nonalcoholic version of Pemberton's French Wine Coca. It was marketed as "Coca-Cola: The temperance drink", which appealed to many people as the temperance movement enjoyed wide support during this time. The first sales were at Jacob's Pharmacy in Atlanta, Georgia, on May 8, 1886, where it initially sold for five cents a glass. Drugstore soda fountains were popular in the United States at the time due to the belief that carbonated water was good for the health, and Pemberton's new drink was marketed and sold as a patent medicine, Pemberton claiming it a cure for many diseases, including morphine addiction, indigestion, nerve disorders, headaches, and impotence. There is no doubt in scientific circles that Coca Cola and other fizzy sugary drinks are partially responsible for the obesity, Type 2 Diabetes around the world, now knowing this bit of history of its origin by a CONFEDERATE GENERAL would help those people, especially those have near or distant connection with slavery and also any of the marginalized, to quit drinking Coca Cola. This would be a good advice to Mexicans who gulp gallons of that stuff per day..

WHAT DOES NEW ZEALAND HAVE THAT THE USA YANKS DO NOT HAVE ?

There are several lessons from New Zealand’s pandemic response. Rapid, science-based risk assessment linked to early, decisive government action was critical. Implementing interventions at various levels (border-control measures, community-transmission control measures, and case-based control measures) was effective. Prime Minister Jacinda Ardern provided empathic leadership and effectively communicated key messages to the public — framing combating the pandemic as the work of a unified “team of 5 million” — which resulted in high public confidence and adherence to a suite of relatively burdensome pandemic-control measures. Future lessons for New Zealand include the need for stronger public health agencies that can better assess and manage potential threats and for greater support for international health organizations, notably the World Health Organization.

IN THE UNITED STATES UNFORTUNATELY INDIVIDUAL FREEDOM TO DO WHATEVER THEY WANT IS GIVEN PRIORITY OVER THE COLLECTIVE WELFARE OF THE STATE.

A college in the NE USA decided to open with 50% of the students, on conditions of Masks, social distancing and compulsory stay at school dorms to reduce the risk of communal spreading. The incoming students were to sign an honour pledge but unbeknown to the college administration, many students had rented houses so that they can continue their partying. The death of the Greatness of America USA as we know it has begun.  As Yuval Noah Harari points out in his latest book, we have to take responsibility not only our nation states but as the planet as a whole if we are to avoid more pandemics, climate disaster and nuclear proliferation ..

SARS-CoV-2 Infection Among Community Health Workers in India Before and After Use of Face Shields

The transmission of severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) is believed to be predominantly through respiratory droplets from infected persons in close proximity to uninfected persons,1 although airborne transmission may also play a role.2,3 Face shields have been proposed to prevent transmission in the community,4 but data are lacking. We describe transmission in a community setting before and after the use of face shields.

Results

Before face shields, 62 workers (40 women) visited 5880 homes with 31 164 persons. From the 5880 homes visited, 222 persons tested positive for SARS-CoV-2, between May 4 to May 13. Twelve workers (19%) were infected during this period. Eight developed symptoms (fever, cough, sore throat, myalgia, and anosmia) and 4 were asymptomatic. The 12 infected workers were moved to care centers. Four developed desaturation and mild breathing difficulty and were treated with oral hydroxychloroquine and oxygen therapy; all 4 recovered. Contact tracing of the workers who tested positive identified 14 van drivers, who were monitored. All were asymptomatic and tested negative between day 7 and 10 after contact with the workers.

After face shields, 50 workers (previously uninfected) continued to provide counseling, visiting 18 228 homes. Among the counseled, 118 428 persons, 2682 subsequently tested positive for SARS-CoV-2. No worker developed asymptomatic or symptomatic infection.

A couple of days ago, Israel was about to trial a breath test which has 95% accuracy. and ready in seconds. todays good news number one is about diagnostic advances in Cancer.



The second one is the follow up to the wonderful diplomatic news of the relationship opening between UAE and Israel..
Sudan which harboured anti-Israelis terrorist groups Hezbollah and Hamas under the now jailed dictator leader, has been preparing to open relationships with Israel.
Warms my heart..Israel can help in so many ways, the struggling democracy of Sudan and also with Medical care and sharing the exciting advancements.. I would love to go Sudah as part of an Israeli team to help Sudanese doctors and clinics..

THE PERMANENT EPIDEMIC IN USA. OBESITY WELCOME FOREIGNERS, COME AND GET FAT

WHERE ARE THOU HEADED ..OBESITY AMONG AMERICANS

 

According to the U.S. Centers for Disease Control and Prevention, 42 percent of people in the country were obese in 2017/2018—a rise from 30 percent in 1999/2000.

 

To answer whether losing weight between early adulthood and middle age is linked to a lower risk of dying later in life, the researchers studied 24,205 adults from the U.S. who took part in the National Health and Nutrition Examination Survey between 1998 through 2015.

 

Only 0.8 percent of the obese participants in the study were able to lose weight and become overweight.


 

Co-author JoAnn Manson, professor of medicine at Harvard Medical School, said in a statement: "Although this study focused on preventing premature deaths, maintaining a healthy weight will also reduce the burden of many chronic diseases such as hypertension, diabetes, heart disease, and even cancer."


 

Two important points

Obesity is INCREASING  now at 42 % of the population.

This is regardless of the plethora of Functional Medicine Functionaries, Nutritional Nabobs, Gurus of all sorts of plagiarized eastern practices in addition to the increasing number of Nurse Practitioners, Physicians Assistants, Doctors devoted to Diabetes…

All related to MONEY, generating money and as a Nigerian paediatrician practicing in a strip mall claimed: specializing in Personal Wealth Transfer.

The second point: less than ONE PERCENT of the obese were able to loose weight and become Overweight.. Tragic indeed.

Who are the fattest people in the United States? They are the same people with very high rate of Diabetes and  Heart disease in the United States.

Obesity and Abdominal Obesity among Indian Americans

§  For the same body mass index (BMI), percentage body fat is 4% to 5% lower for blacks and Pacific Islanders but 7-8% higher for Asian Indians compared to Europids.1, 2

§  The prevalence of obesity among Asian Indians depends on the criteria used. A BMI >25 is classified as obese for Asian Indians and all Asians by the Asia Pacific and/or Indian criteria whereas, a BMI > 30 is classified as obese for all Europids regardless of nationality.3-5

§  Significant underestimation of overweight and obesity occurs when standard criteria is used.6, 7 Two cross sectional studies of Asian Indians in the US showed a mean BMI of 25 to 26.6, 7 Using the standard criteria (BMI >30) only 11% to 17% were obese, which increased to 50 -70% when Asia pacific and/or Indian criteria (BMI >25) was used.6, 7 Thus Asian Indians in the US have the highest prevalence of obesity. Another 25% were overweight (BMI 23-25).6, 7

§  A similar underestimation of abdominal obesity occurs when the standard (Europid) waist circumference criteria is used (>102 cm for men and >88cm for women) instead of Asia pacific and/or Indian criteria (>90 cm for men and >80cm for women).3-5

§  Using standard criteria, the prevalence of abdominal obesity among Asian Indians in the US was 23% which increased to 60% when using Asia Pacific and/or Indian criteria, compared to 58% in whites, 75% in Hispanics, and 76% in blacks.8 This prevalence is slightly higher than the 49% reported from India.9


Add to this :  Filipino Americans have an overweight plus obesity rate, one of the highest in USA at around 78%

The Cardiovascular disease patterns in South Asians is one of diffuse rather than local disease 

If you ask a group of people showing photographs of the same person with normal weight or overweight, the overweight person is always picked as being "American". Subconsciously many Asian-American may want to look and feel "American"


 

Just ask around your Functional Medicine Functionaries and other self-proclaimed purveyors of Diseases in the USA.. and see how many of them know this fact?

While at it, ask your Nutritionist to explain to you the difference between High Fructose Corn Syrup and Maltodextrin and Dextrose and corn starch.

You will see.

 

One of the reasons why Americans fail to lose weight, despite all that is available to them in terms of advice and technology is that

Technology, medications, advice with good intentions, any amount of exercise cannot fight the chemicals that are entering the bodies of the Americans

More Processed the food, greater the amount chemicals entering the body.

The last time I looked it up, Americans in USA , regardless of their immigration status, recent arrivals or arrived in the 16th Century, eat about ONE POUND OF CHEMICALS EVERY THREE DAYS…

 

And now you can wonder why Americans keep on becoming FATTER and FATTER by the day and any immigrant who in their eagerness to become “American”, will begin to look like the 42% who are obese.

Truly you do not need Supplements or Probiotics if you EAT food. 

Food possibly is the greatest instigator of Inflammation in the body of the Americans, closely followed by Mental Health problems including Depression.  Not all people who has queries about their mental health are mentally ill.. but there is hardly anyone to ask… in this system where everything is not easy to access.

 

Jim BB one of my UmonHon Indian teachers once told me about the health care providers:

When the pain is in the body, they immediately jump in and attend to you

(just go anywhere and say that you have severe pain in the chest and see for yourself)

But when the pain is in the heart, no one can help you ..


 

If you are an American of Asian descent whether from Korea or Sri Lanka , please try to maintain a body weight of BMI 22 Kg/M2.. At 25 kg/M2, you are on your way to City of Diseases..
